I hereby authorize my minor child to participate in the activities held at the Multidisciplinary Sports Complex of L’Ancienne-Lorette (CSMAL) or at the Bruno Verret Arena in St-Etienne (ABVSE) during his/her time at the "Extreme Power Skating" hockey school.
In the event of an injury to my child, I authorize "Extreme Power Skating" and the "École de patin personnalisé de Québec" to contact ambulance services to provide the best possible care.
This document also releases Jonny Murray, Guillaume Ouellet, Charles-William Gagné, as well as the employees of the "Extreme Power Skating" hockey school and the "École de patin personnalisé de Québec", as well as CSMAL and ABVSE and their respective employees, from any liability for injuries that may occur to my child during his/her time with us.
